All About You
Under general direction, the HIV Case Manager provides medical case management services, including treatment adherence, to offer a range of client-centered services that link clients with health care, psychosocial, and other services. The position coordinates and follows-up on medical treatments; ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems; participates in learning and educational programs. Coordinates core and supportive services provided as well as non-Ryan White funded services and community providers; coordinates client-centered services that link clients with health care, psychosocial, and other services; assists clients with accessing other public and private programs for which they may be eligible; coordinates and follows-up on medical visits, labs, and assessment of viral load suppression; provides treatment adherence counseling; advocates for client-specific and personal support.
In addition, the Lead role performs the following duties:
• Assign new referrals for CM from Intake Coordinator and the clinic. This includes reviewing referral documentation, assessing caseloads for assignment, and providing communication with referral source and case manager. This includes documentation in SCOUT.
• Assign transfers for CM from Intake Coordinator and/or other Lead CMs or CM supervisors. This includes those transfers into our agency and out to other agencies. This includes reviewing client chart, assessing caseloads for assignment, and providing communication with referring and receiving case managers. This includes documentation in SCOUT.
• Contact for clients who present with issues with their case managers to help with resolution of the concern, or work with client to transfer using OTP (Optional Transfer Policy) • Provide feedback and support to CMs about client issues, training questions, systemic issues, etc.
• Lead CM is copied on all communications in SCOUT regarding client issues, transfers and referrals.
• Attend meetings with STL DOH regarding CM grant.
• Complete monthly invoice reporting for SLU Grants department. This includes accessing reports with monthly encounter durations, demographics on clients receiving services, and service units.
• Complete quarterly/end of year reports for STL DOH as required for Part A funding.
• Coordinate portions of the Part A Request for Proposal for case management this year and in past years.
• Maintain monthly transportation listing for reimbursements and work with STL DOH to pick up transportation in-person. Lead CM must verify and distribute transportation reimbursements to CMs.
• Review and distribute monthly Medicaid spenddown listings, open referral listings (6CSM), no insurance listings, ACA client listings, etc. provided by RCMS. Lead CM discusses with CMs to follow up with clients when needed. • Contact Illinois CM system staff to verify, when necessary, if client is enrolled in Illinois CM system to avoid duplication of services.
• Prepare agenda and facilitate meetings 2 times per month with CM staff, Clinical Director, and outside agencies that are working currently with SLU.
• Attend monthly QI meeting and complete/monitor those tasks as assigned to CM staff.
• Audit CM charts to ensure MO DHSS guidelines are met and notes meet quality standard. • Attend monthly regional supervisor meetings with CM supervisors and Lead CMs in region.
• Coordinate orientation for new staff (this was a role in the past, not sure if this will change with current new staff)
• Complete on and off-boarding process for exiting CMs.
• Build and maintain relationships with outside organizations that support our work and our clients.
• Prepare annual performance reviews of the HIV Case Managers.
• Approve vacation/sick leave requests from CMs to make sure that adequate staffing is available.
• Collaborate with CMs to write CM staff internal policies and procedures.
• On call for clinic from 8am-1pm approximately 3-4 times per month.
• Attend monthly CM seminars, quarterly trainings, committee meetings and additional meetings required throughout the year.
• Various administrative tasks in regards to facility concerns, supplies, or operational concerns.

MINIMUM QUALIFICATIONS
Education and experience equivalent to:
Bachelor’s degree; supplemented with one (1) year of related work experience in a healthcare environment. State license or ACSW preferred.
NOTE
This position will migrate to SSM Saint Louis University Hospital as of 3/1/24. SSM Health will become the employer. Annual salary will be unchanged, but each employer has their own benefits package.
